Club Policy

I hereby seek membership of Parkville United A.F.C. I understand that the committee of Parkville United A.F.C. shall not be responsible, either financially or otherwise for any loss or damage to my property or for any injury sustained by me whilst playing for the club. This form must be accompanied by the Annual Membership Fee of €340.00, which includes the cost of Personal Accident Insurance and league registration fees. N.B. A fee of €5 to cover club running costs and ongoing infrastructure and coaching development is included in this fee. Please note the Carlow League form will still need to be signed separately from this form.

I hereby grant permission for my child on this membership form to seek membership of Parkville United A.F.C. I understand that the committee of Parkville United A.F.C. shall not be responsible, either financially or otherwise for any loss or damage to his/her property or for any injury sustained by him/her whilst he/she is playing for the club. I understand that my child must comply with the current guidelines as detailed in the FAI safer return to training and play protocols regarding COVID 19 and adhere to the instructions of the designated COVID officer. I understand that I must not send my child to training or games if they are showing any of the symptoms of COVID19, specifically:

Your child must stay home if they 

- Have been in contact wity someone with COVID19 or a probable case of COVID19 in the last 14 days,

-  Have been overseas or exposed to someone with COVID19 in the last 14 days,

- Have flu like symptoms or are feeling unwell. 

You must

- Check with your GP if your child is in a high risk category and ask if your child is medically fit to train.
